Many people feel fine at first, then hunger surges later In the STEP 1 extension, participants regained about two thirds of prior weight loss in the year after stopping A 2026 BMJ meta-analysis estimated average regain of roughly 0.4 kg per month after stopping weight-loss medications, with return toward baseline projected within 1.5 to 1.7 years The same meta-analysis found no evidence that more intense behavioural support during treatment meaningfully changed the rate of regain after stopping Weight regain is often accompanied by reversal of cardiometabolic improvements, not just scale change Summary Stopping semaglutide should be understood as a biology handoff
